Vouchers not used within this time will be forfeited with no entitlement to refund or exchange. Note all advertised treatment times include a period of consultation. *Dinner style or service is subject to change without notice. **Access to the indoor pool and fitness suite should be booked with hotel reception on arrival at the resort.

• If the goods purchased total less than the value of the gift voucher, any balance will be left as a credit for you with Formby Hall Golf Resort & Spa, and will be redeemed against subsequent orders.
• Formby Hall Golf Resort and Spa gift vouchers remain valid for 12 months from date of
purchase. Expiry dates cannot be extended.
• Gift card must be present in order to redeem goods or services at Formby Hall Golf Resort & Spa.
• Lost or damaged gift cards cannot be replaced.
• Gift cards cannot be used in conjunction with members levy cards.
• Can be redeemed at spa and golf shop.
• In addition can be used against any of our day or break offers and promotions.
• Cannot be used in the food and beverage outlets across the resort.
